This service supports individuals in maintaining independence and dignity outside of institutional settings.
Key Features:
Personalized Care: Caregivers, whether family or non-family members, receive training to deliver tailored support services based on the participant’s assessed needs.
Daily Stipend: Caregivers receive a daily stipend based on the level of care required:
Level 1: $55.00 per day
Level 2: $75.00 per day
Level 3: $106.75 per day
Support Services: Includes assistance with Activities of Daily Living (ADLs), community activities, and necessary appointments.
Quality Assurance: Regular home visits by registered nurses and caregiver coaches ensure ongoing support and monitoring.
