Pricing

Basic Home Visits

Designed for clients who have already met with the provider. Each session includes a thorough re-assessment to identify any changes that may require updates to the emergency care plan. We also ensure all necessary documentation is completed and properly filed to confirm that the session took place.

$150 per visit

Additional Charges

• Initial Home Visits: $200 per visit
  The first visit includes the development of a comprehensive emergency care plan tailored to the individual’s needs.  

• Emergency Care Plan Updates: $25 minimum charge
  If an emergency care plan needs to be updated during a home visit (for clients who have already met with the provider), it will be reviewed and adjusted as necessary. For complex updates requiring more than 30 minutes, an additional $25 fee will be added.

• Caregiver Training
  If caregiver training is requested during a home visit, additional charges will apply based on the type and length of the session:

  • • 1-Hour Basic Skills Training: $25 per session
      Covers essential caregiving skills like personal hygiene, mobility assistance, and supporting daily living activities.

    • 1-Hour HCBS Training: $50 per session
      Focuses on care delivery in home-based settings, covering topics such as the Home and Community Settings Rule, BDS Core Competencies, Life Course Framework, and Person-Centered Planning (topics will be customized based on caregiver needs).

    • 1-Hour Custom Training: $50 per session
      Tailored to specific topics such as Alzheimer’s care, dementia support, or other specialized caregiving challenges.

    • Resource Navigation Training: $60 per session
      Guidance on connecting to community resources, financial assistance programs, and help with Medicaid Waiver applications to ensure caregivers have the resources they need.

    • 2-Hour CPR Training: $75 per session
      Includes American Heart Association BLS certification, valid for 2 years.

• Travel Beyond 5 Miles
  An additional $2 per mile will apply for travel beyond a 5-mile radius from 46805. This charge is calculated one-way only.
